How we deliver it
Meaningful days look different for everyone. Staff may accompany individuals to recreation, volunteering, shopping, or visits with friends, with transportation and coaching aligned to personal preferences and HCBS settings principles.
What this includes
Funding & eligibility
Many individuals access this service through Kansas Home and Community-Based Services (HCBS) waivers. We work directly with Managed Care Organization (MCO) care coordinators and case managers to align authorizations, Individual Support Plan (ISP) goals, and documentation requirements. Not sure if you qualify? Submit a referral and we will help route the conversation. No clinical details required in the first message.
